Note: This problem has now been fixed. I would like to thank Microsoft for the quick turn around for this approvals issue!

Today I had a look at the approval steps available in Microsoft Flow. I created a flow that created an approval step.

Then I ran my flow and found the approval notification in outlook.

However in the above dialog I didn’t supply the reason. Hey, it is optional anyway!

{{approve.value}}

Now when I look at my email gain the Comments are {{approve.value}}

Wow, something is going wrong here!

It still gets worse. The output from the action gives me the same token:

I’ve reported this to Microsoft already, so hopefully this will be fixed soon. However if you find that you use comments in your flows you might want to check for this token text and replace it with nothing in your flows.
